Viewer 20@All: During the process of “Open Existing/Add New Consumer” Step 7 (page 15) states “Be cautious when entering information in an existing consumer record not to change any data entered by another user. Then jump to Step 9” – the issues is the consumer was identified, but all the boxes are grey and no changes are allowed. Changes are requires, so should I have access to make necessary changes or do I add a new consumer with corrected information?

Viewer 20@All: Thank you. I will take a look at that. SAMS wasn't allowing me to add a new assessment at that time, the options were not available…but I did not trying saving and going back.

Consumer wants to change provider

Viewer 17@All: How do we apply for a consumer who wants to transfer from one agency to another?

Jim O (AGD Support)@All: Viewer17 : the consumer may wish to move to another AFC or GAFC provider. If you are the 2nd provider, you should gather the documentation and submit. The new provider needs an initial determination from Coastline.

Viewer 16@All: @ Jim O's re: Viewer 17's question, regarding transferring from one agency to another. A new assessment only had to be done if the previous agency was more than 6 months old, is this still the case?

Jim O (AGD Support)@All: Sandra: if you have submitted new users, they should receive 2 emails within a day or so. If your users had Field test access, you will not get additional emails — your existing account was changed from Field Test to Production. your same passwords and security questions should apply

Duplicate provider organization record

Viewer 6@All: I have logged in and tried to set up the defaults, etc. Under provider our agency is listed twice, two different ways. Is one correct or will both of them work? Northeast ARC and Northeast Arc, Inc.

Jim O (AGD Support)@All: Yes, we will always try to acknoeldge such requests – there's no notification that comes from Harmony on a termination request.

Timing & Age of Clinical Documentation

Viewer 14@All: if the MDS is valid for 90 days – do we have that amount of time to get the RFS, PSF, cover letter, etc to Coastline?

Jim O (AGD Support)@All: @viewer 16 — per MaryEllen. If a consumer is determined eligible by Coastline, then services may commence within 6 months. If there is a lag, and more than six months passes, then a new Initial Determination is needed.

Viewer 14@All: so how much time do we have once MDS is completed to send info to Coastline

Viewer 6@All: My point is that the PSF is your physician's order to do the MDS in the first place. Is this wrong?

Jim O (AGD Support)@All: Viewer 6 — the PSF must be submitted with the MDS to Coastline. Since the MDS has a shelf-life of 90days from the onsite assessment, the clinical documents should be gathered as quickly as possible. Everything must be submitted as a complete bundle

Viewer 6@All: There needs to be a physician's order to have AFC according to the regs.

Jim O (AGD Support)@All: clarification to my comment above to @viewer 16 — per MaryEllen. If a consumer is determined eligible by Coastline, then services *must* commence within 6 months of the Eligibility Notification. If there is a lag, and more than six months passes before services begin, then the provider must get a new Initial Determination.
