If you or someone at your agency is unable to login to the Mediware Portal to open SAMS, please send a SECURE email to AGD.Support@massmail.state.ma.us. When contacting us, a clear, relevant & unique email subject line is most helpful. DO NOT contact Mediware directly as they will not be able to assist you.
The email should include the following information:
- Your organization name, consistent with provider name in SAMS.
- Your name (as the requestor)
- Your phone number
- First and Last Name for User Login Account (if different from you)
- Email address for User Login Account (if different from you)
- Telephone Number for User Login Account
Sample Subject Lines where AGD Example Care Services, Inc. represents your company name:
- Terminate User – AGD Example Care Services, Inc.
- Address Change – AGD Example Care Services, Inc.
- Name Change – AGD Example Care Services, Inc.
- New User Request – AGD Example Care Services, Inc.
- Login Trouble – AGD Example Care Services, Inc.
- Password Problem – AGD Example Care Services, Inc.
