Then jump to Step 9” – the issues is the consumer was identified, but all the boxes are grey and no changes are allowed. Changes are requires, so should I have access to make necessary changes or do I add a new consumer with corrected information?

Viewer 20@All: Thank you. I will take a look at that. SAMS wasn't allowing me to add a new assessment at that time, the options were not available…but I did not trying saving and going back.
Consumer wants to change provider
Viewer 17@All: How do we apply for a consumer who wants to transfer from one agency to another?
Jim O (AGD Support)@All: Viewer17 : the consumer may wish to move to another AFC or GAFC provider. If you are the 2nd provider, you should gather the documentation and submit. The new provider needs an initial determination from Coastline.

If your users had Field test access, you will not get additional emails — your existing account was changed from Field Test to Production. your same passwords and security questions should apply

Duplicate provider organization record

Timing & Age of Clinical Documentation

Jim O (AGD Support)@All: @viewer 16 — per MaryEllen. If a consumer is determined eligible by Coastline, then services may commence within 6 months. If there is a lag, and more than six months passes, then a new Initial Determination is needed.

Jim O (AGD Support)@All: Viewer 6 — the PSF must be submitted with the MDS to Coastline. Since the MDS has a shelf-life of 90days from the onsite assessment, the clinical documents should be gathered as quickly as possible. Everything must be submitted as a complete bundle

Jim O (AGD Support)@All: clarification to my comment above to @viewer 16 — per MaryEllen. If a consumer is determined eligible by Coastline, then services *must* commence within 6 months of the Eligibility Notification. If there is a lag, and more than six months passes before services begin, then the provider must get a new Initial Determination.
